Locum Hospital Social Worker – ARCH Team, Stockport Council

Rate: £30 per hour

Location: Stepping Hill Hospital & Community-Based (Pathway 1 and 2)
Hours: Full-time


We have an exciting opportunity for two qualified Hospital Social Workers to join the ARCH Team in Stockport. Based at Stepping Hill Hospital, with involvement in Pathway 1 and 2, you will work as part of a fast-paced, supportive, and person-centred hospital discharge team.

This role is central to ensuring smooth transitions from hospital to home or bed-based Discharge to Assess (D2A) settings, in line with the “Home First” ethos. You will contribute to effective hospital discharge planning, working closely with health professionals and the wider multidisciplinary team (MDT), while also supporting Stockport’s Transfer of Care hub as needed.


Key Responsibilities:

  • Work from hospital wards to assess and support timely, safe discharges.

  • Complete Care Act assessments and strength-based support planning.

  • Support people in Pathway 1 and 2 settings, including same-day discharges.

  • Liaise with health colleagues and community services to promote best outcomes.

  • Support Reablement and provider teams in community-based work.

  • Participate in duty rota, MDT meetings, and team-based support planning.

  • Apply safeguarding procedures and ensure robust risk management.

  • Maintain accurate records and comply with statutory responsibilities.


Requirements:

  • Qualified Social Worker with registration to Social Work England.

  • Experience in hospital discharge and multidisciplinary working.

  • Excellent understanding of the Care Act 2014 and safeguarding processes.

  • Able to work under pressure and manage a varied and dynamic caseload.

  • Strong communication, problem-solving and teamworking skills.

  • Experience supporting hospital pathways and D2A processes is essential.
