Role: Adults Senior Social Worker

Pay: Up to £38 per hour
Contract: 3-month ongoing (with potential for extension)

Hammersmith and Fulham Council is recruiting an experienced Senior Social Worker to join their Community Response Team, delivering timely, person-centred interventions to adults in the borough. This is a frontline role focused on hospital discharge, prevention of admission, and community-based support to promote independence.

Role Highlights:

• Up to £38 per hour (umbrella)

• 3-month ongoing contract, with strong potential to extend

• Hybrid working – 2 to 3 days per week in the office

• Work within a supportive, fast-paced, and well-structured team

Responsibilities:

• Complete strength-based assessments and person-centred care planning

• Support safe and timely hospital discharges

• Prevent unnecessary hospital admissions through early intervention

• Collaborate with health professionals, families, and community services

• Lead on safeguarding, complex cases, and statutory responsibilities

Requirements:

• At least 3 years’ post-qualified experience in adult social care

• Registered with Social Work England

• Solid knowledge of the Care Act, Mental Capacity Act, and safeguarding protocols

• Experience in hospital discharge, reablement, or short-term intervention services
