Are you a dedicated and experienced Social Worker looking for a rewarding role in a hospital setting? Hammersmith and Fulham are seeking a skilled professional to join the team at Charing Cross Hospital.
 

Role: Hospital Social Worker
Location: Charing Cross Hospital, Hammersmith and Fulham
Rate: £35.50 per hour (agency position)
Contract: 3 months+
Work Arrangement: 2 days a week in the office
 

Key Responsibilities:

• Undertake assessments for patients in a hospital setting, ensuring timely and safe discharges.
• Work closely with healthcare professionals, patients, and families to develop robust care and support plans.
• Provide advice and support to patients regarding available services and community resources.
• Ensure compliance with the Care Act 2014 and other relevant legislation in all aspects of social work practice.
• Maintain accurate and timely case records.
   

Requirements:

• Social Work England (SWE) registration.
• Experience in hospital social work, discharge planning, or related roles.
• Strong understanding of safeguarding, mental capacity assessments, and care planning.
• Ability to work effectively within a multidisciplinary team.
