Job Title: Locum Hospital Social Worker
Location: Windsor and Maidenhead
Pay Rate: £32 – £34 per hour (umbrella)
Contract: Locum / Full-Time

We are seeking an experienced Locum Hospital Social Worker to join the Adult Social Care team in Windsor and Maidenhead. This role offers a competitive hourly rate and the opportunity to work in a dynamic hospital environment, supporting safe discharges and ensuring patients receive the right care and support.

Key Duties:

• Carry out assessments within the hospital setting to identify care needs and plan safe discharge pathways.

• Work closely with hospital discharge teams, health professionals, and external agencies.

• Undertake safeguarding enquiries in line with Care Act 2014 requirements.

• Prepare clear, accurate records, reports, and support plans.

• Manage a busy caseload with efficiency and professionalism.

Requirements:

• Social Work England registration (essential).

• Previous hospital discharge or adult social care experience.

• Knowledge of Care Act 2014, MCA, and safeguarding adults.

• Ability to start at short notice (desirable).
