About the Role:

We are seeking a dedicated and experienced Qualified Social Worker to join our Locality Team in Berkshire. This part-time role, working 18.5 hours per week, offers flexibility while ensuring you stay connected with the team and service users by attending our Slough offices two to three times a week.

You will be responsible for assessing, planning, and delivering social care services to meet the needs of vulnerable adults within the community. The role focuses on providing tailored, strengths-based interventions, safeguarding individuals, and working collaboratively with a multidisciplinary team to ensure that residents receive the best possible support.

Key Responsibilities:

• Conduct Care Act 2014 assessments to identify eligible social care needs.
• Develop person-centred care plans that support the independence, wellbeing, and choices of service users.
• Monitor and review care plans, ensuring that they adapt to the changing needs of individuals while delivering cost-effective solutions.
• Safeguard vulnerable adults, following relevant policies and procedures.
• Manage a varied and complex caseload with the ability to make informed decisions in partnership with service users, carers, and other professionals.
• Work collaboratively with other teams, including healthcare, housing, and community partners, to provide holistic care.
• Attend the Slough office 2-3 times per week to maintain close working relationships with the team and attend necessary meetings and supervision.

About You:

• Social Work England registered, with up-to-date qualifications.
• Experience working with adults and older people in a community-based setting, ideally within a Locality Team.
• Strong understanding of the Care Act 2014 and other relevant legislation.
• Excellent communication, assessment, and problem-solving skills.
• Ability to manage a complex caseload, make sound decisions, and advocate for service users' rights.
• Collaborative, with a commitment to working as part of a multidisciplinary team.
