Our Older People and Physical Disabilities Service is a key part of the local authority's mission to provide high-quality, strengths-based statutory social care. We are located in the vibrant heart of Southwark, offering a convenient office location directly opposite Queens Road train station and well-connected by bus routes.

The Role:

As a Social Worker in our Case Management team, you will:

• Handle complex social work interventions for older adults and people with physical disabilities, including those with mental health and substance misuse needs.
• Undertake assessments, Safeguarding enquiries, and unscheduled reviews under the Care Act 2014.
• Work on mental capacity issues and Court of Protection applications as required.
• Collaborate in multi-disciplinary teams for Continuing Health Care processes.

Requirements:

• Experience: Minimum of 3-4 years post-qualifying statutory experience, ideally with a background in working with adults with complex needs in the community.
• Registration: Must be registered with Social Work England.
