Flexible Working | Hybrid Model | Mental Health Review Team
Rochdale’s Adult Mental Health Service is seeking an experienced Adults Social Worker to join their Review Team. This is a fantastic opportunity to work in a team that delivers targeted mental health reviews and support planning across both health and adult social care pathways.
This is a full-time agency role, offered on a hybrid basis to support flexible working. The successful candidate will have extensive experience in both Community Mental Health Teams (CMHT) and Adult Social Care services, alongside strong assessment, review, and caseload management skills.
Key Responsibilities:
Complete Care Act assessments and support planning
Carry out timely reviews of mental health care packages
Apply the principles of the Mental Capacity Act and safeguarding adults
Work jointly with CMHT professionals, carers, and external providers
Maintain accurate records in line with statutory duties
Promote independence, wellbeing, and person-centred care
Practice Model:
Rochdale’s Mental Health Review Team works across social care and health interfaces to ensure residents receive timely and appropriate reviews and interventions. This is a role where robust multi-agency collaboration and a person-centred mindset are essential.
Essential Requirements:
Qualified Social Worker registered with Social Work England
5+ years’ post-qualified experience in adult social work
Experience in both CMHT and Adult Social Care
Strong understanding of Care Act, MCA and safeguarding procedures
Ability to work independently and manage a varied caseload
Full UK driving licence and access to a vehicle
Experience using Liquid Logic (desirable but not essential)
