Hybrid Working | Adult Mental Health Review Team | Flexible Start Dates

Rochdale’s Adult Mental Health Service is seeking a highly experienced Adults Social Worker to join their dedicated Review Team. If you’re confident in mental health practice and have a background in both CMHT and Adult Social Care, this role offers the opportunity to work within a supportive, forward-thinking service.

This is a full-time agency role offering hybrid working. The successful candidate will carry out targeted reviews and contribute to delivering high-quality, person-centred mental health care under the Care Act framework.

Key Responsibilities:

  • Carry out Care Act assessments and mental health reviews

  • Update and develop support plans in line with assessed needs

  • Apply safeguarding principles and the Mental Capacity Act

  • Work collaboratively with CMHT professionals and health partners

  • Maintain timely and accurate case records

  • Promote person-centred, strengths-based approaches in all interventions

Practice Model:
The Review Team plays a vital role in delivering effective care planning and intervention within Rochdale’s integrated health and social care model. The work is fast-paced but rewarding, focused on maintaining and improving outcomes for adults with mental health needs.

Essential Requirements:

  • Qualified Social Worker registered with Social Work England

  • Minimum 5 years’ post-qualified experience in adult social work

  • Previous experience in both CMHT and Adult Social Care settings

  • Strong understanding of the Care Act, MCA, and safeguarding procedures

  • Ability to work independently with a complex caseload

  • Full UK driving licence and access to a vehicle

  • Familiarity with Liquid Logic (desirable but not essential)
