A local authority in South London is seeking a dedicated and experienced Social Worker to join our Adults Learning Disability Team. This role offers the opportunity to make a meaningful impact on the lives of adults with learning disabilities, ensuring they receive the support, care, and advocacy needed to lead independent and fulfilling lives.
Key Responsibilities:
- Manage a diverse caseload of adults with learning disabilities, conducting comprehensive assessments and developing personalized care plans.
- Work closely with service users, their families, and carers to provide tailored support that meets their unique needs and promotes their well-being.
- Collaborate with a range of professionals, including healthcare providers, housing services, and community organizations, to deliver holistic care and support.
- Ensure compliance with statutory requirements and local authority policies, maintaining accurate and timely records.
- Participate in multi-disciplinary meetings, advocating for the best interests of service users and ensuring their voices are heard in decision-making processes.
- Provide expert advice and support to colleagues and contribute to the continuous improvement of services within the team.
About You:
- Qualifications: Must be registered with Social Work England and hold a recognized social work qualification.
- Experience: Proven experience working with adults with learning disabilities or within a similar field, with strong case management skills.
- Skills: Excellent communication, assessment, and report-writing skills, with the ability to manage complex cases effectively.
- Location Flexibility: While the role requires 2-3 days in the office, we offer flexible working arrangements to support your work-life balance.
