Are you a passionate and dedicated Social Worker, looking to make a difference in the lives of individuals in the Slough community? We are seeking a qualified Social Worker to join our dynamic Locality Team in Slough, Berkshire.

Key Responsibilities:

• Facilitate and support service users to undertake supported self-assessments, empowering them to make informed choices about the services they receive.
• Conduct thorough assessments of risk, need, and capacity, ensuring that appropriate actions are taken to safeguard individuals when necessary.
• Deliver high-quality social work practice in line with the Care Act, Mental Capacity Act, Continuing Health Care, and Safeguarding guidelines.
• Work collaboratively with individuals, families, and other professionals to develop comprehensive care plans that reflect the needs and preferences of service users.

Essential Requirements:

• Social Work England registration (must be current).
• In-depth knowledge of key legislation including the Care Act, Mental Capacity Act, Continuing Health Care, and Safeguarding practices.
• Strong understanding of social work theory, ethics, and relevant legislation.
• Experience in conducting assessments, managing risk, and working with individuals to develop care plans.
• Excellent communication skills and ability to work both independently and as part of a multidisciplinary team.
