Senior Practitioner – Adults Case Management Team – London

Location: London Borough of Hackney
Pay/Salary: Up to £36.00 per hour (Umbrella)
Office Working: Hybrid – minimum 2 days per week in the office
Contract: Locum

We are currently working with the London Borough of Hackney to recruit an experienced Senior Practitioner for their Adults Case Management Team. This is a community-based service working with adults who have longer-term and more complex social care needs.

This is a supervisory Senior Practitioner position, providing oversight and support to approximately five Social Workers. You'll provide clear case direction, quality assurance and professional supervision while helping ensure statutory work is legally compliant, completed to a high standard and progressed within appropriate timescales.

The wider team works with adults experiencing needs relating to older-age frailty, dementia, mental health, autism, learning needs, self-neglect and hoarding, as well as individuals with No Recourse to Public Funds (NRPF).

Duties

Providing professional supervision and case direction to approximately five Social Workers.
Offering regular formal and informal supervision and practice support.
Quality assuring assessments, support plans and other statutory Social Work activity.
Ensuring practice is legally compliant and completed to a consistently high standard.
Monitoring case progression and ensuring timely throughput of work.
Identifying training and development needs within the team.
Supporting practitioners with complex decision-making, risk and case management.
Providing oversight of Care Act assessments, carers assessments and Mental Capacity Assessments.
Supporting work involving Continuing Healthcare assessments and funding arrangements.
Promoting high-quality relationship-based interventions with adults and their families.
Contributing to the overall development and effective running of the Case Management Team.
Supporting good practice across complex areas including self-neglect, hoarding and NRPF.

What you need

Social Work Qualification.
Social Work England registration.
Enhanced DBS (provided by us).
Significant experience within statutory Adult Social Care.
Previous Senior Practitioner, Advanced Practitioner or supervisory experience.
Strong knowledge of the Care Act and Mental Capacity Act.
Experience providing supervision, case direction and professional guidance to Social Workers.
Strong quality assurance skills and knowledge of statutory Social Work requirements.
Experience managing or overseeing complex community-based Adult Social Care cases.
Confidence supporting practitioners with risk, safeguarding and complex decision-making.
Ability to work collaboratively and contribute to the wider development of the service.
