Key elements of the Leaving Care Service include:

1. Personal Advisors: These advisors offer tailored support and guidance, helping care leavers to develop and implement their Pathway Plans, which outline their goals and the steps needed to achieve them.

2. Housing Support: Assistance is provided to help young people secure suitable accommodation, whether through council housing, private rentals, or supported living arrangements.

3. Education and Employment: The service supports care leavers in accessing further education, vocational training, and employment opportunities, aiming to equip them with the skills and qualifications needed for their chosen careers.

4. Health and Well-being: Ensuring that care leavers have access to healthcare services and support for their mental and emotional well-being is a priority. This includes facilitating connections to local health services and providing information on healthy living.

Requirements:

• Must be registered with Social Work England.
