Job Title: Social Worker – Adults Localities Team

Introduction
Sutton Council is seeking an experienced Social Worker to join their Adults Localities Team. This is a great opportunity to work in a forward-thinking and supportive environment focused on empowering adults to live independently and safely within their communities.
The team works with adults across the borough with a wide range of needs, including older people, individuals with physical disabilities, and those with long-term or complex health conditions.
This role offers a competitive rate of £38.50 per hour and a hybrid work pattern, allowing a healthy balance between home and office-based working.
Key Responsibilities
- Manage a varied caseload of adults under the Care Act 2014, undertaking assessments, support planning, and reviews.
- Apply a strengths-based, person-centred approach to promote independence and wellbeing.
- Work collaboratively with health professionals, voluntary organisations, and community partners to ensure effective support planning.
- Carry out safeguarding enquiries (Section 42) and risk assessments to protect vulnerable adults.
- Ensure all case recording, assessments, and plans meet statutory and quality standards.
- Participate in team duties and contribute to the delivery of timely, proportionate responses.
Candidate Requirements
Essential:
- Qualified Social Worker with Social Work England registration
- Proven experience within Adult Social Care, ideally in a community or locality team
- Strong understanding of the Care Act 2014, Mental Capacity Act 2005, and safeguarding procedures
- Excellent assessment, communication, and report-writing skills
- Ability to work effectively in a hybrid environment (3 days office-based)
Desirable:
- Experience of integrated working with NHS and voluntary sector partners
- Familiarity with Mosaic or similar case management systems
