It is your responsibility to look after your own health and safety and that of others who may be affected by what you do (or do not do).

Please review the following Health & Safety Information:

  • Familiarise yourself with the health & safety policy, fire & evacuation and first aid arrangements when on a new assignment
  • Ensure you participate in any required induction / training and supervision in relation to health & safety
  • Ensure you cooperate with others on health and safety, and do not interfere with or misuse anything provided for your health, safety or welfare
  • Ensure any protective equipment provided to you is used as required and instructed
  • Only carry out duties and responsibilities for the post you have been hired to do, refraining from undertaking any other duties which you may not be
    a) competent to undertake and
    b) could endanger your own health, safety or welfare

If, during the course of a temporary assignment, the Client wishes to employ me directly, I acknowledge that Sheldon Phillips will be entitled to charge the client an introduction / transfer fee, or to agree an extension of the hiring period with the Client (after which I may be employed by the Client without further charge being applicable to the Client).

I understand that if any false information has been provided within this application, it may result in the cancellation of my registration with Sheldon Phillips.

I will notify Sheldon Phillips of any changes that may affect my eligibility to work, and notify of any changes to my personal details.
