Position: Social Worker
Team: Hospital Discharge & Post-Assessment Discharge
Contract: Locum
Hours: 37 hours per week
Pay Rate: £30 per hour
Location: Calderdale Royal Hospital & Huddersfield Royal Infirmary
Working Pattern: Hybrid with on-site presence required for hospital and community work
Sheldon Phillips are working with Calderdale Council to source an experienced Social Worker to join their Hospital Discharge and Post-Assessment Discharge Team. This service supports safe and timely hospital discharges, completing assessments across two key acute sites and following up with community-based interventions to ensure adults receive the right level of support. The role involves both hospital ward work and community visits across Calderdale and neighbouring areas.

Varied caseload covering hospital, post-discharge, and wider community tasks
Strong MDT relationships with NHS colleagues
Duties and Responsibilities:
Complete Care Act assessments for patients ready for discharge
Lead and coordinate safe discharge planning with hospital MDTs
Undertake community visits and follow-up assessments post-discharge
Manage safeguarding concerns and complete enquiries under Section 42
Work collaboratively with health partners, therapy teams, and community providers
Record work to a high standard within statutory timeframes
Support adults with a wide range of needs, ensuring proportionate and outcome-focused plans
Requirements:
SWE Registration
Social Work qualification
Strong post-qualified experience in hospital discharge, community teams, or adult social care
Knowledge of the Care Act, Mental Capacity Act, and Continuing Healthcare processes
Ability to work confidently within multi-agency settings
Flexible, proactive, and organised approach
