Position: Qualified Social Worker
Team: Leaving Care (14–17 year olds)
Contract: Locum (3 months +)
Hours: 37 hours per week
Pay Rate: £33.88 per hour
Location: Stoke-on-Trent (Hybrid Working Available)
Start Date: ASAP
Working Pattern: Monday to Friday

Sheldon Phillips are working with Stoke-on-Trent City Council to find a dedicated Social Worker to join their Leaving Care Team, supporting young people aged 14–17. The team plays a crucial role in helping young people prepare for independence and adulthood. You’ll be supported by a genuinely lovely and approachable team manager and join a warm and well-established team environment.

Ongoing support and regular supervision
Duties and Responsibilities:
Support 14–17 year olds as they transition into adulthood and independent living
Create and implement tailored pathway plans for each young person
Work closely with education providers, health professionals and housing services
Undertake statutory visits and ensure young people’s voices are central to planning
Record work clearly and maintain compliance with statutory requirements
Requirements:
Social Work England (SWE) registration
Qualified Social Worker
Experience in Leaving Care, LAC, or working with adolescents
Strong communication and relationship-building skills
Ability to work independently and as part of a team
