Position: Qualified Social Worker
Team: Leaving Care (14–17 year olds)
Contract: Locum (3 months +)
Hours: 37 hours per week
Pay Rate: £33.88 per hour
Location: Stoke-on-Trent (Hybrid Working Available)
Start Date: ASAP
Working Pattern: Monday to Friday

Sheldon Phillips are working with Stoke-on-Trent City Council to find a dedicated Social Worker to join their Leaving Care Team, supporting young people aged 14–17. The team plays a crucial role in helping young people prepare for independence and adulthood. You’ll be supported by a genuinely lovely and approachable team manager and join a warm and well-established team environment.

Duties and Responsibilities:

• Support 14–17 year olds as they transition into adulthood and independent living

• Create and implement tailored pathway plans for each young person

• Work closely with education providers, health professionals and housing services

• Undertake statutory visits and ensure young people’s voices are central to planning

• Record work clearly and maintain compliance with statutory requirements

 

Requirements:

• Social Work England (SWE) registration

• Qualified Social Worker

• Experience in Leaving Care, LAC, or working with adolescents

• Strong communication and relationship-building skills

• Ability to work independently and as part of a team
