Senior Social Worker – Transitions Team – Medway

Location: Medway Council
Salary: up to £47,435 per annum
Working Pattern: Full Time
Contract: Permanent
Essential Car User: Valid UK driving licence and access to own vehicle required

We are currently working with Medway Council to recruit an experienced Senior Social Worker for their Transitions Team. This is a specialist service supporting young people aged 17–25 as they transition from Children's Services into Adult Social Care.

You'll play a key role in ensuring young people experience a smooth and positive transition into adulthood, working closely with families and professionals across Children's Social Care, SEND, Housing and Health. Alongside managing your own complex caseload, you'll provide professional supervision, oversight and guidance to Social Care staff and help drive quality and practice improvement across the service.

Duties

  • Managing a complex caseload of young people aged 17–25 transitioning into Adult Social Care.
  • Using strengths-based and personalised practice to promote independence and positive outcomes.
  • Providing professional supervision, coaching and mentoring to Social Care staff.
  • Providing professional advice and oversight on complex cases and higher levels of risk.
  • Working closely with Children's Social Care, SEND, Housing, Health and other partner agencies.
  • Completing Mental Capacity Assessments and participating in Best Interest meetings.
  • Completing Court of Protection DoL applications and renewals where required.
  • Completing Section 42 Safeguarding Enquiries and acting as a Designated Safeguarding Officer.
  • Considering alternative funding arrangements with the ICB, including Continuing Healthcare and Section 117 aftercare.
  • Leading quality assurance, audits and practice improvement activity.
  • Developing practice guidance, training materials and best-practice tools.
  • Representing Medway Council at key meetings, panels and multi-agency groups.
  • Ensuring practice complies with the Care Act, Mental Capacity Act and Mental Health Act.

What you need

  • Social Work Qualification.
  • Social Work England registration.
  • Enhanced DBS (provided by us).
  • Strong experience within statutory Adult Social Care.
  • Experience working with young people transitioning from Children's to Adult Social Care.
  • Strong knowledge of the Care Act, Mental Capacity Act and Mental Health Act.
  • Experience completing Mental Capacity Assessments and Best Interest work.
  • Strong Adult Safeguarding experience, including Section 42 enquiries.
  • Confidence managing complex cases and higher levels of risk.
  • Experience providing professional supervision, mentoring or practice guidance to other staff.
  • Knowledge of CHC and Section 117 funding arrangements.
  • A valid UK driving licence and access to your own vehicle.
