Social Worker – Children with Disabilities Team

Pay Rate: £39.04 per hour

About the Role

London Borough of Merton is seeking an experienced Social Worker to join its Children with Disabilities (CWD) Team.
You will support children and young people with complex needs, ensuring they receive high-quality assessments, tailored support packages, and strong safeguarding intervention where required.

This is a specialist, relationship-focused role working closely with families, health professionals, schools, and multi-agency partners.

Key Responsibilities

Complete Children Act assessments for children with disabilities and complex needs.
Manage a specialist caseload including CIN, CP, and short breaks support.
Undertake safeguarding enquiries and risk assessments.
Develop personalised care and support plans for children and families.
Work closely with education, paediatric services, and community providers.
Ensure the voice of the child shapes planning and intervention.
Maintain accurate, timely records and prepare high-quality reports.
