We are seeking a dedicated and experienced Qualified Social Worker to join our dynamic Integrated Localities and Hospital Team in East London. In this role, you will undertake targeted reviews of care packages, including double-handed care calls, early reviews of hospital discharges, and domestic care calls (cleaning, shopping, and housework) for individuals with Physical Disabilities and Learning Disabilities. Your primary goal will be to improve care outcomes while contributing significantly to budgetary savings.

Key Responsibilities:

• Conduct comprehensive reviews of existing care packages to ensure they are proportionate to the presenting needs and level of risk.
• Perform early reviews of hospital discharges to facilitate smooth transitions and appropriate care planning.
• Assess and revise double-handed care calls, promoting independence and wellbeing through positive risk-taking.
• Evaluate and recommend the use of equipment or Assisted Technology to reduce the need for double-handed care where feasible.
• Ensure that care packages are equitable and mindful of budget pressures, while still meeting the necessary standards of care.
• Collaborate with multidisciplinary teams to develop and implement individualized care plans that reflect best practices and innovative approaches.

Essential Requirements:

• A recognised Social Work qualification and current registration with Social Work England.
• Proven experience in adult social work, particularly within the areas of Physical Disabilities and Learning Disabilities.
• Strong understanding of proportionate care principles and positive risk-taking.
• Ability to critically evaluate care packages and implement cost-effective solutions.
• Excellent communication and interpersonal skills to work effectively with service users, families, and professional colleagues.
• Commitment to promoting independence and enhancing the quality of life for individuals.

Desirable Skills:

• Experience working within an integrated health and social care setting.
• Familiarity with the use of Assisted Technology in care delivery.
• Ability to manage complex cases and make informed decisions under pressure.
