Team: Locality - Team 2 
Pay:  up to £36 per hour
Contract: Full-Time Locum

We are seeking a skilled and dedicated Adults Qualified Social Worker to join our Locality Team 2 in South-West London. This role involves carrying out comprehensive reviews, assessments, and safeguarding activities to ensure the well-being of adults in our community. Key Responsibilities:

• Conduct reviews of care and support plans to ensure they meet the needs of service users.
• Assess needs under the Care Act 2014 and develop appropriate care plans.
• Address safeguarding concerns and take necessary actions to protect vulnerable adults.
• Perform mental capacity assessments to determine individuals’ ability to make decisions.
• Conduct home visits to assess living conditions and provide support.
• Carry out carers assessments to identify the needs and support required by caregivers.
• Review placements to ensure they provide adequate care and support.

Requirements:

• Qualified Social Worker with Social Work England.
• 2 to 3 years of experience working in adult social care settings.
• Strong understanding of the Care Act 2014 and its application.
• Experience in safeguarding, mental capacity assessments, and carers assessments.
• Excellent assessment and care planning skills.
• Ability to work independently and as part of a team.
• Strong communication and interpersonal skills.
• Commitment to promoting the well-being and rights of adults in the community.
