Are you a compassionate and dedicated Social Worker with experience in adult social care? Join our vibrant team in West London as a Qualified Social Worker for our Adults Review Team. You will play a vital role in conducting reviews and assessments to ensure the well-being and support of adults in our community.

As a Qualified Social Worker on our Adults Review Team, your responsibilities will include:

• Conducting face-to-face reviews and assessments with adults in need of support and assistance.
• Completing thorough assessments to determine the level of need and appropriate support services required.
• Providing support, guidance, and advocacy to adults to promote their independence and well-being.
• Completing accurate and timely documentation, including assessment write-ups, ensuring compliance with policies and procedures.
• Managing a workload of 5 reviews per week, prioritizing tasks effectively to meet deadlines.
