Social Worker – Children with Disabilities Team – Midlands

Salary: £45,777 - £50,091

We are currently working with Herefordshire Council to find an experienced Social Worker for their Children with Disabilities team in Childrens Services. With a stable management team and a mostly permanent workforce, Herefordshire are looking for someone with solid experience to be able to pick up a caseload and support their service, Herefordshire also offer Sponosrship.

Duties

• Managing caseload for Children with Disabilities requiring assessment of their needs
• Carrying out Single Social Work Assessments of the children as well as the assessment of their carers (Carers’ Assessments) to identify the needs of children with disabilities, the needs of carers and providing support to address identified needs.
• Participating in CTRs (Care and Treatment Reviews) meetings in managing the health care of children with disabilities, with the aim of reducing admissions and unnecessarily lengthy stays in hospitals.
• Identifying and recommending support to lessen identified risks and exploring appropriate respite support package in meeting the children’s specific needs.
• Convening and participating in Initial and Review Health Assessments of looked after children and young people.
• Convening and participating in statutory looked after children’s review meetings.
• Preparing reports such as PEP, EHCP, Resource Allocation Panel (RAP)and LAC paperwork for Statutory review Looked after Children meetings.
• Participating in Pathway Assessments and Pathway Meetings and completing pathway plans in preparing young people for Transition into adulthood.
• Liaising and developing close working relationships with other professionals and agencies to ensure clients receive services in accordance with assessed needs and within multi-agency framework.
